GP practices Edith Cavell Surgery and Streatham High Practice have reduced blood pressure control between BAME and white patients through a 12-month project.

Between 27th Oct 22 and 2nd February 23, Streatham AT Medics facilitated our first Cohort of monthly Chronic pain peer support sessions at 2-8 Gracefield Gardens Health and Social Centre. We had a ratio of up to 30 patients attending the monthly sessions and over 45 attend the RTW-empowered relief workshop.


Following feedback received from patients across the PCN we launched our next cohort of chronic pain peer support sessions on Friday 2nd of June to run once per month until 1/12/23. We aim to have a clinician/ pharmacist and Dietitian attend a session so patients can ask questions about certain foods and medications.



Health & Wellbeing Fair for our Black community

On the 25th of May 23 Streatham PCN in Partnership with Streatham AT Medics PCN hosted a Black Health & Wellbeing Fair at Christ Church, Streatham.

The event had 350 plus in attendance, with support from the following stalls: Mental Health, Prostate Cancer, Carers4Carers, Social Prescribing help desk, AT Beacons Project (healthcare screening), Healthwatch, Lambeth Health & Welling being bus, Yoga Point, Food for Purpose, Crosslight money advice, Chronic Pain support.



Some of the qoutes from patients were very heartwarming see below:


“ It’s nice to feel acknowledged as a black male in the health system, I rarely feel seen being here today makes me feel in touch with my community”
“Since my husband passed away I’ve felt so lonely. I never heard about social prescribing before so I was very interested that my surgery has this service. I signed up to receive a referral for my social prescriber, I’m looking forward to getting support”


By being able to work collaboratively we were able to reach so many patients Lambeth.

The positive impact the day created for our patients and the local community was phenomenal. Over 40 new proactive social prescribing referrals, so much productive learning on improving health and wellbeing.

Meet with Tobacco Dependence Specialist, Mark, & your Social Prescribers & Health Coaches to discover more about what’s making it hard for you to quit and to develop new coping strategies.

There is a free monthly menopause peer support session taking place locally please refer to the flyer below.


The next Thriving Streatham event will be on Thursday the 27th of July on Streatham Common. We will be focusing on Green social prescribing and the positive impact it can have on mental health wellbeing.
